Sciatica.

Sciatica describes pain that travels along the path of the sciatic nerve — typically from the lower back, through the buttock, and down one leg. It's often accompanied by tingling, numbness, or weakness, and can range from a dull ache to sharp, debilitating pain.

At ALGN, we assess the root cause of sciatic nerve irritation before treatment begins. Common causes include disc herniation, piriformis syndrome, spinal stenosis, and facet joint dysfunction — and each requires a different approach.

Treatment typically includes spinal manipulation, joint mobilisation, soft tissue therapy, and progressive nerve mobilisation exercises. We also address contributing factors such as posture, movement patterns, and any lifestyle factors that may be maintaining the problem.

Neck Pain.

Neck pain is one of the most common musculoskeletal complaints we see at ALGN — particularly in patients with desk-based roles or those who spend extended time looking at screens. It ranges from acute, sharp onset to chronic stiffness and restricted movement accumulated over months or years.

We assess neck pain comprehensively: examining the cervical spine, the thoracic spine, the shoulder girdle, and the muscles that support them. Pain in the neck is rarely isolated to the neck itself.

Treatment may include cervical and thoracic joint manipulation, deep tissue massage, dry needling where appropriate, and progressive strengthening exercises to support lasting improvement. Where nerve referral into the arm is present, we also address this within treatment.

Postural correction and ergonomic advice are provided as standard — the best treatment is one that addresses the environment generating the problem, not just the symptom.

Lower Back Pain.

Lower back pain is the leading cause of disability worldwide — and one of the most mismanaged. At ALGN, we apply a classification-based approach to lower back pain: understanding whether your presentation is discogenic, facet-related, muscular, or nerve-mediated changes what we do significantly.

We use chiropractic spinal manipulation, joint mobilisation, soft tissue therapy, and targeted exercise rehabilitation based on your specific diagnosis. We don't apply generic low back protocols — each programme is built from your assessment findings.

For acute lower back pain, early movement guided by evidence is key. For chronic lower back pain, we focus on load management, movement quality, and progressive capacity building. Both benefit from the integration of chiropractic and massage that ALGN offers.

Postural Tension.

Postural tension is the cumulative result of sustained positions — hours at a desk, poor workstation setup, repetitive asymmetrical tasks — loading certain muscles and joints beyond their comfortable capacity. It typically presents as diffuse aching in the neck, upper back, or shoulders that builds through the day.

Treatment at ALGN for postural tension combines deep tissue massage to release the over-loaded tissues with mobilisation of the joints that have become stiff, and corrective exercise to build the capacity to sustain better positions for longer.

Crucially, we also provide ergonomic assessment and practical guidance on workstation positioning, movement breaks, and load management. Without addressing the environment, manual therapy for postural tension is a temporary fix at best.

Sports Injuries.

Sports injuries are a broad category that includes everything from acute strains and sprains through to overuse injuries, stress-related presentations, and post-surgical rehabilitation. ALGN works with athletes at all levels — from weekend runners to competitive club sport participants.

Our approach to sports injuries is the same as all our treatment: assessment-first. We identify the structure involved, the mechanism of injury, contributing factors, and — critically — when and how to load the tissue to facilitate optimal recovery.

We treat common sports injuries including hamstring strains, rotator cuff issues, ankle sprains, runner's knee (patellofemoral pain), Achilles tendinopathy, and ITB syndrome. We work closely with patients to manage load during their return to sport and reduce the risk of reinjury.

Headaches & Migraines.

Cervicogenic headaches — headaches originating from the neck — are frequently misclassified and undertreated. They are caused by dysfunction in the upper cervical joints and surrounding musculature, and respond well to chiropractic treatment when correctly identified.

At ALGN, we distinguish between cervicogenic headaches, tension-type headaches, and migraine presentations during assessment. The treatment approach differs significantly between these, and accurate classification is essential for effective management.

For cervicogenic and tension-type headaches, we use a combination of upper cervical joint manipulation, deep tissue massage to the suboccipital muscles and upper trapezius, and postural correction. Many patients experience significant reduction in headache frequency and intensity within a small number of sessions.

For patients with migraines, we work as part of a broader management approach — addressing any musculoskeletal triggers and contributing factors while supporting the patient's wider care plan.

Frequently asked questions.

I've had sciatica for months — is it something you can actually fix?

In most cases, yes. Sciatica can feel endless, but the underlying cause is usually identifiable and treatable. We look for the specific source of the nerve irritation — whether that's a disc, a joint, or tight musculature — address it directly, and give you the tools to prevent it recurring. Most patients with genuine sciatica see real improvement within a handful of sessions.

My back pain keeps coming back. Why isn't it going away?

Recurring pain is almost always a sign that the underlying cause hasn't been fully addressed — only the symptoms have been managed. Each flare-up tends to reinforce the pattern. What we do differently is classify the root cause of your pain first, then build treatment around that. Resolving the pattern, not just the episode, is the goal.

My neck is always stiff and tight — will massage actually help?

Almost certainly, yes — especially when combined with the right exercises and some attention to why it's tight in the first place. Sports massage can release the muscle tension and restore movement, but if the underlying cause is postural or joint-related, we'll address that too. We often combine massage and chiropractic for neck presentations.

I've been told I just have to live with the pain — is that really true?

Sometimes, but far less often than people are told. Many of our patients arrive having been dismissed elsewhere, and find that the right assessment and targeted treatment makes a significant difference. We'll give you an honest picture of what's achievable for your specific condition — and if we genuinely can't help, we'll say so and point you somewhere that can.
